Clinical Social Worker 1

Job Description Under the supervision of the Clinical Supervisor for the Citywide Supportive Housing Program, the Clinical Social Work I/II (CSW I/II) works with San Franciscans living in Permanent Supportive Housing who struggle with mental and physical health, substance use issues and housing retention to collaborate with community providers and link residents to appropriate outpatient services and provide housing stabilization to prevent homelessness. The CSW I/II will carry a caseload of 25 residents from one of the DISH supportive housing sites (Camelot, Le Nain, Empress, Windsor, Star, Pacific Bay Inn) and provide individual treatment including case management services, psychotherapy, crisis intervention, 5150 assessments, substance use treatment, psychoeducation, group treatment, community events, intake, assessment, service planning and consultation, and other duties as assigned. The CSW I/II maintains client records according to California, City, and County requirements; participates in an interdisciplinary treatment team; attends weekly staff meetings; and completes all required training sessions. The CSW I/II coordinates with property management, DPH Nursing, and other service providers to develop treatment strategies, advocacy, and coordination with agencies and families; tracks client contacts in Epic and One Systems; conducts referrals and liaison with hospitals and clinical medical providers; and performs other related duties. Client care focuses on a client‑centered approach that supports individuals with disabilities to attain optimal psychological, social, and vocational outcomes and maximizes housing retention through case management and advocacy. The position requires the ability to lift 50 lbs, walk, and climb stairs. Responsibilities Provide individual treatment (case management, psychotherapy, crisis intervention, assessment, psychoeducation, group treatment) to a caseload of 25 residents. Maintain accurate client records in accordance with state, city, and county regulations. Participate in interdisciplinary treatment team meetings and bi‑weekly staff sessions. Coordinate with property management staff, DPH Nursing, hospitals, clinics, and community providers to develop treatment plans and referrals. Track client contacts and referrals in Epic and One Systems. Provide advocacy and support to promote housing stability and prevent homelessness. Document and assess outcomes, prepare reports, and meet deadlines. Engage in ongoing training and professional development. Partner with Grief Services and other interdisciplinary teams as needed. Qualifications Master’s degree in Social Work, Psychology, or a related field from an accredited university. Registered with the California Board of Behavioral Science as an Associate Clinical Social Worker (ASW) or Intern Marriage Family Therapist (IMFT) for CSW I; or required credentials for licensure for CSW II. Two years working with severely mentally ill adults, the homeless, and dual‑diagnosed populations. One year experience in community‑based case management with outreach to SRO hotels and scattered site housing. Ability to work cooperatively, communicate effectively, and handle high‑stress, high‑volume environments. Excellent written and verbal communication skills. Knowledge of medical and psychiatric terminology. Demonstrated commitment to anti‑racism, social justice, and cultural competency. Demonstrated reliability and excellent attendance. Physical ability to lift 50 lbs and climb stairs. Preferred Experience with intensive outreach and engagement of clients with long histories of mental illness and homelessness. Ability to secure safe and affordable housing for clients with behavioral and substance abuse problems. Demonstrated problem‑solving skills in complex, mandated situations (e.g., reporting to Adult Protective Services). Required Licenses & Certifications BBS Licensure, LCSW, LPCC, MFT.
