Seniority Healthcare is seeking a part-time Nurse Practitioner or Physician Assistant to join our team in New City, NY for 16 hours a week. In this role, you will provide comprehensive care to a high-need, high-risk geriatric population. The ideal candidate has a background in Internal Medicine, Family Medicine, or Geriatrics, and can work independently while collaborating with a multidisciplinary team. This position offers a balance of direct patient care and remote responsibilities-such as prescription refills, lab reviews, and patient communication-allowing for a flexible and rewarding schedule. - Conduct thorough physical exams and wellness assessments for seniors
- Follow up on care plans to ensure the best possible outcomes
- Propose and manage treatment plans for chronic and acute conditions
- Maintain accurate patient records and manage appointments effectively
- Educate patients on preventative care, treatments, and healthy lifestyle choices
- Collaborate with a supportive team dedicated to holistic care
- Completion of an accredited NP or PA program
- At least one year of experience in geriatrics, internal medicine, or family practice
- Strong communication, active listening, and analytical skills
- Compassionate and committed to providing patient-centered care
- Motivated to serve a geriatric population with complex care needs
- Ability to work 16 hours a week, 2 days onsite at an assisted living facility.
