Facility Hospice Care Coordinator – LPN

to support hospice patients residing in skilled nursing facilities (SNFs) and assisted living facilities (ALFs). This hybrid position combines nursing care with proactive communication and care coordination to help keep patients, families, facility staff, and the Mountain Valley hospice team informed and connected. What You'll Do: Coordinate Care:

Round routinely in SNFs and ALFs, build relationships with facility staff, and participate in facility care plan meetings as appropriate. Keep Families Connected:

Provide proactive communication and follow-up with patients and families, particularly following changes in condition or significant events. Provide Nursing Support:

Complete PRN and supplemental LPN visits, reinforce hospice education, and recognize and communicate changes in patient condition to the RN Case Manager. Support the Care Team:

Identify communication or service concerns and ensure they are communicated to the appropriate clinical team member or leader for follow-up. Your Schedule: Monday - Friday, 8 AM - 5 PM. Hybrid schedule with travel to skilled nursing and assisted living facilities. Qualifications Valid North Carolina Licensed Practical Nurse (LPN) license. Hospice, home health, skilled nursing, or assisted living experience preferred. Exceptional interpersonal and communication skills with a compassionate and patient-centered approach. Strong organization and follow-through with the ability to manage needs across multiple facilities. Ability to work independently and collaboratively as part of the interdisciplinary hospice team. Reliable transportation and ability to travel throughout the assigned service area.
