This remote, full-time role partners with members to address social barriers, guide them to health activities, and document progress in the Reema App. You will manage a caseload, enroll members, and coordinate referrals while collaborating with the Care Team Manager and multidisciplinary staff to uphold HIPAA standards and deliver compassionate, community-centered care. #J-18808-Ljbffr Reema Health
