Social Worker (MSW)

Role: Hospice Medical Social Worker Responsibilities include: Providing emotional, social, financial, and resource support to patients and families facing serious illness and end-of-life care. Key duties involve conducting social casework, assessing financial situations, facilitating referrals, participating in interdisciplinary team conferences, and connecting families with community resources. Ensuring the use of patient-centered approaches such as the 4Ms (What Matters, Medications, Mentation, Mobility) and providing age-friendly care are essential.
