Physical Therapist

Job Title: Home Health Physical Therapist Location: Spring Hill, TN Job Type: Full Time Overview We are looking for a Physical Therapist to join our Home Health team in the Spring Hill area and surrounding communities. Responsibilities Assess and evaluate patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Determine a treatment plan based on assessment findings and perform interventions aimed at improving and enhancing patient well‑being. Evaluate patient progress and report outcomes to supervising nurse and/or physician. Assist the physician in developing the therapy plan of care. Observe and report the patient’s response to treatment and any changes in condition. Treat patients to relieve pain, develop and restore function, and maintain maximum function using acceptable standards of practice. Inform patients, family/caregivers, and the health care team about the plan of care, functional limitations, and progress toward goals. Qualifications Current PT license in the state where services are rendered. Current CPR certification. Driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation. Ability to reliably commute to multiple patient residences daily within the assigned geographic area.
