Occupational Therapist Home Health

Day shift, 5 days per week (08:00 - 17:00).

Assignment Details

Minimum of 6 home visits per day, with potential for additional visits based on management discretion. Potential weekend shifts of 1-2 per contract.

Requirements

• 1 year of home health experience required.
• OASIS training preferred.
• Valid driver's license and access to a vehicle for home visits.
• Certifications: BLS required.
• COVID vaccination required or an exemption form on file.
• Flu shot required or declination with mandatory mask-wearing while at work.
• Experience with home skilled visits for various patient needs including med/surg, cardiac, ortho, post-surgical, respiratory, neuro, and trauma patients.

Responsibilities

• Conduct patient assessments and develop treatment plans.
• Provide therapeutic interventions in home settings.
• Document patient progress and adjust care plans as necessary.
• Collaborate with healthcare team members to coordinate care.
• Educate patients and families on treatment goals and techniques.
