Role Overview

As a Hospice RN, you will serve as more than just a nurse; you will be a clinical leader, advocate, and a steady source of support for patients and their families.

Your responsibilities will include leading care plans, managing symptoms, and ensuring that each patient receives personalized, comfort-oriented care that aligns with their goals and preferences. With each visit, you will provide not only clinical expertise but also reassurance, clarity, and compassion.

Your Responsibilities

• Lead Patient-Centered Hospice Care:
  • Conduct thorough assessments of patients and their families.
  • Create and oversee individualized care plans focused on comfort.
  • Administer skilled nursing care with an emphasis on pain and symptom management.
• Serve as the Care Coordinator:
  • Act as a case manager, ensuring coordinated care across the interdisciplinary team.
  • Collaborate with physicians, social workers, chaplains, and aides.
  • Monitor patient conditions and adjust care plans to ensure comfort.
• Support Patients & Families:
  • Provide education, emotional support, and guidance throughout the care journey.
  • Assist families in navigating complex decisions with empathy and clarity.
  • Establish meaningful, trusting relationships during deeply personal moments.
• Drive Quality & Compliance:
  • Maintain accurate and timely documentation.
  • Participate in interdisciplinary team meetings and care planning.
  • Support adherence to quality and compliance standards.

Qualifications

• Current RN license (or eligibility for compact license).
• Graduate from an accredited nursing program.
• At least one year of clinical nursing experience.
• Strong clinical judgment and a compassionate approach to care.
• Ability to manage a caseload independently.
• Excellent communication skills, especially in sensitive situations.
• Basic proficiency in electronic medical records (EMR).
• Current CPR certification.
• Valid driver's license and reliable transportation.
