Role Overview This position involves the coordination and delivery of physical therapy services, developing and implementing individualized treatment plans based on patient evaluations. The therapist assesses patients, creates treatment programs, and educates patients and caregivers on exercises and home programs. Key Responsibilities Evaluate patients through various data sources and develop personalized treatment strategies. Provide direct therapy services across different settings, ensuring care aligns with patients' physical, emotional, and intellectual needs. Document evaluations, treatment plans, progress notes, and communicate regularly with patients, families, and interdisciplinary team members. Supervise and oversee physical therapy assistants, technicians, and students to ensure safe and effective patient care. Participate in ongoing professional development through education, workshops, and staying current with best practices. Qualifications And Skills Active license as a Physical Therapist in the practice state. Strong communication skills and the ability to work effectively within a team.
