Registered Nurse (RN)

Job Overview

We are seeking a dedicated and experienced Registered Nurse (RN) to join our team in a Long-Term Care/SKilled Nursing Facility (LTC/SNF). This position requires working 12-hour night shifts, with responsibilities including providing high-quality patient care, collaborating with interdisciplinary teams, and maintaining efficient patient flow.

Key Responsibilities

• Deliver nursing care to patients in accordance with established standards and protocols.
• Monitor patient health and assess their needs to provide the best possible care.
• Administer medications and treatments as prescribed.
• Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans.
• Maintain accurate and detailed patient records in the electronic medical record system.
• Provide education and support to patients and their families regarding health conditions and care plans.
• Participate in continuous quality improvement initiatives.

Qualifications

• Active Registered Nurse (RN) license.
• Current Basic Life Support (BLS) certification.
• Minimum of 2 years of nursing experience in a Long-Term Care/SNF or similar setting.
• Ability to work every other weekend and holiday as needed.
• Strong communication, organizational, and interpersonal skills.

Work Environment

This role is situated in a facility with a capacity of 58 beds and employs a patient-to-nurse ratio of 1:15. Employees will be required to perform location-specific orientation upon hire and are expected to participate in a shadowing program where feasible.

Patient Population

The primary patient population consists of geriatric individuals receiving long-term care, including those with various chronic conditions and disabilities.

Additional Notes

• Orientation includes 1 hour of building orientation before the first shift.
• Shifts may be assigned based on variable scheduling, including both 8-hour and 12-hour shifts.
