The area's extensive park system, recreational facilities, and community events create a welcoming environment for professionals looking to build a long-term home base or gain strong, resume-building experience in a dynamic healthcare market.In this role, you will provide skilled Physical Therapy services to adult and geriatric patients in their homes, focusing on improving mobility, safety, independence, and quality of life. Typical conditions include post-acute and post-surgical care, orthopedic and joint replacements, neurologic conditions such as stroke or Parkinson's disease, balance and fall risk issues, cardiopulmonary diagnoses, and generalized deconditioning. You will conduct thorough assessments, develop individualized treatment plans, implement therapeutic interventions, and provide education to patients and caregivers on exercises, equipment use, and home safety modifications.A typical day includes traveling between patient homes, performing 4 to 6 visits per day depending on visit type and geography, documenting care in the EMR, and coordinating with an interdisciplinary team that may include nursing, occupational therapy, speech therapy, social work, and primary care providers. You will perform gait training, transfer training, therapeutic exercises, balance and strength programs, and home safety evaluations, while continually reassessing progress and adjusting care plans as needed. Patient caseloads are designed to be manageable, allowing time for thorough care, documentation, and travel.You can expect a schedule that generally follows daytime hours, with options for some flexibility based on patient needs and organizational policies. Weekend or holiday coverage may be requested on a rotational basis. While patient ratios are not expressed in a traditional inpatient sense, visit expectations are communicated clearly and supported by scheduling staff who help optimize routes across Garden City and nearby communities to reduce unnecessary drive time.This position offers the opportunity to sharpen your clinical skills in the patient's natural environment, which is an increasingly valued experience in modern healthcare.
