Position Summary Oceana Healthcare Center is seeking a detail-oriented Licensed Vocational Nurse (LVN) MDS Nurse to join our team full-time. This position supports timely, accurate completion of Minimum Data Set (MDS) assessments, resident care planning, clinical documentation, and reimbursement processes. The ideal candidate has experience in skilled nursing or long-term care, communicates effectively across disciplines, and is committed to maintaining accurate and compliant resident records. Key Responsibilities Coordinate and complete MDS assessments accurately and within required timeframes. Participate in comprehensive resident assessments and interdisciplinary care planning. Review medical records and clinical documentation for completeness, consistency, and coding accuracy. Ensure care plans reflect each resident's current needs, goals, preferences, and services. Collaborate with nursing, therapy, social services, dietary, medical records, and other members of the interdisciplinary team. Monitor assessment schedules, supporting documentation, and required follow-up. Identify documentation gaps and communicate needed corrections or clarification to the appropriate team members. Help ensure compliance with applicable federal, state, and facility documentation requirements. Support clinical reimbursement, quality-measure review, and care-coordination activities. Maintain resident confidentiality and organized, accurate clinical records. Perform other duties related to MDS coordination and resident care planning as assigned. Qualifications Current California LVN license in good standing. Previous experience in a skilled nursing or long-term care setting preferred. MDS assessment and care-planning experience strongly preferred. Familiarity with current MDS requirements and skilled nursing documentation practices preferred. Strong organizational, time-management, and critical-thinking skills. Excellent written and verbal communication skills. Ability to manage deadlines and collaborate effectively with an interdisciplinary team. Proficiency with electronic health records and clinical documentation systems.
