Case Management Specialist

The Case Management Specialist provides support to youth in transitional housing, assisting with obtaining IDs, vital records, and housing applications. They conduct individual and group sessions focusing on life skills such as budgeting, financial management, employment readiness, and stress reduction. The role includes developing service plans, documenting interactions in electronic systems, and advocating for residents by offering referrals and supportive counseling. Specialists facilitate life skills groups, coordinate monthly house meetings, and track residents' work, savings, and investments. They maintain program records, complete assessments, and ensure compliance with policies. Qualifications Qualifications include a master’s degree in social work or related field (preferred), 2-4 years of youth case management experience, proficiency in Microsoft Office, and experience with housing assistance and skill-based group facilitation. They should possess strong communication skills, cultural competency, and flexibility to work in diverse settings and outside regular hours.
