Description

• Conduct evaluations in both inpatient and outpatient settings while crafting tailored treatment plans
• Deliver specialized physical therapy interventions targeting mobility, strength, stability, and functional recovery
• Collaborate with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation experts to optimize patient outcomes
• Provide education to patients and their families regarding therapeutic goals, discharge plans, and home care exercises
• Maintain comprehensive documentation of evaluations, treatments, and progress following regulatory and facility requirements
• Participate in interdisciplinary care rounds and conferences to ensure seamless patient-centered care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
