Sanford Health & Rehabilitation is seeking an experienced Social Worker to join our team! You will work alongside fellow caregivers to provide high quality patient care.

Responsibilities include:
Assess nature of patients' situations by interviewing and reviewing personal history
Develop and execute individual treatment plans/care plans
Determine appropriate milestones to gauge patient progress
Communicate patient updates to all relevant parties
Facilitate referrals to other healthcare professionals and programs as needed
Maintain accurate documentation
Qualifications:
Previous experience in social work, with a focus on short term and long term care patients
Compassionate and caring demeanor
Ability to build rapport with staff, patients, and their loved ones
Strong leadership qualities
Excellent written and verbal communication skills
