It's one less thing to worry about, so you can focus on your patients.Responsibilities:Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needsDevelop, implement, and update individualized plan of care in partnership with patients and familiesProvide skilled nursing care, including pain and symptom management, medication administration, and comfort measuresEducate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of lifeCoordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care teamParticipate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic careQualifications:Active Registered Nurse (RN) license in the state you work inProficiency with electronic medical records (EMR)Strong organizational, communication, and critical-thinking skillsAbility to work independently while maintaining high standards of careA compassionate, patient- and family-centered approachValid driver's license and reliable vehicle with current insuranceBenefits:Competitive salaryFleet car program with fuel and insurance covered, for business and personal use *Based on eligibilityMedical, dental, and vision insuranceLife insurance, Long term/Short term disability insuranceGenerous PTO package401(k) with company match
