Acute Care Case Management - Discharge Planning

Title: Acute Care Case Management - Discharge Planning

Schedule: Daily caseload: up to 25 patients; Weekend requirement: rotating; typically every other weekend; Orientation: 40 hours

Assignment Details: Contract Length: TBD; Guaranteed Hours: TBD

Requirements: Minimum 2 years of acute care case management/discharge planning experience in a hospital setting; CA state license required (pending accepted for review but must be active on start date); Prior travel experience required; Competence in utilization review, care coordination, and discharge planning; Experience with DME, SNF placement, and ambulance transport; Familiarity with HMOs/IPAs, prior authorizations, DRG, and core measures; Experience with electronic medical records (Epic preferred)

Roles & Responsibilities: Coordinate and facilitate discharge planning processes to ensure smooth transitions of care; Conduct comprehensive assessments and care coordination for patients; Arrange for necessary medical equipment, skilled nursing facility (SNF) placement, and transportation; Participate in family meetings and collaborate with interdisciplinary teams

Compliance / Notes: Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day); RTO: 7 days (may affect assignment/guaranteed hours); Background checks and drug screens may be required
