Title
Acute Care Case Management - Discharge Planning

Schedule
- Daily caseload: up to 25 patients
- Weekend requirement: rotating; typically every other weekend
- Orientation: 40 hours

Assignment Details

Requirements
- Minimum 2 years of acute care case management/discharge planning experience in a hospital setting
- CA state license required (pending accepted for review but must be active on start date)
- Prior travel experience required
- Competence in utilization review, care coordination, and discharge planning
- Experience with DME, SNF placement, and ambulance transport
- Familiarity with HMOs/IPAs, prior authorizations, DRG, and core measures
- Experience with electronic medical records (Epic preferred)

Roles & Responsibilities
- Coordinate and facilitate discharge planning processes to ensure smooth transitions of care
- Conduct comprehensive assessments and care coordination for patients
- Arrange for necessary medical equipment, skilled nursing facility (SNF) placement, and transportation
- Participate in family meetings and collaborate with interdisciplinary teams

Compliance / Notes
- Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
- RTO: 7 days (may affect assignment/guaranteed hours)
- Background checks and drug screens may be required
