Our high-functioning interdisciplinary team supports this goal by providing fully integrated medical and behavioral health services, community resources, and home-based care in a collaborative and supportive environment. Position OverviewTheRegistered Nurse is a crucial member of the PACE Interdisciplinary Team (IDT), providing PACE program participants who experience a wide range of chronic healthcare needs with coordinated nursing care in their own homes. The RN demonstrates expertise in the assessment, case management, and implementation of participants' overall plans of care. They also utilize evidence-based nursing interventions to support these comprehensive care plans, as well as collaborate with the enrollment team in performing initial assessments of participants' home care needs, such as medication reconciliation and warm hand-offs to other IDT members. In addition, the RN communicates the current status of participants to the IDT, including new problems and/or problems indicating a significant change in status, in order to inform best practices for continued care.QualificationsCurrent or conditional Massachusetts RN licensure requiredAmerican Heart Association BLS Provider certification required; Phlebotomy skills preferredB.S. in Nursing preferredReliable transportation to make daily home visits in the CHA PACE service areaOne to three years of home care experience preferred Highly developed geriatric assessment and clinical skillsExperience at a PACE plan, working on an interdisciplinary team, etc. is desiredFluency in Spanish, Haitian Creole, Portuguese, etc. is desiredClinical experience with geriatric, behavioral health and/or disabled populations is desiredPlease note that the final offer may vary within the listed Pay Range, based on a candidate's experience, skills, qualifications, and internal equity considerations.
