What You'll Do Perform in-home assessments to address activities of daily living (ADLs), instrumental ADLs, and home safety. Create personalized treatment plans focusing on energy conservation, use of adaptive equipment, and functional strengthening for daily tasks. Schedule and conduct patient home visits based on your preferred caseload and availability. Work with a supportive team to ensure a holistic approach to each patient's plan of care.
