Behavioral Health Case Manager: Advocate & Care Planner
COPE Community Services
COPE Community Services is hiring a Behavioral Health Case Manager to provide support for members on their recovery journey. This role includes developing and updating service plans, monitoring progress, and advocating for members to ensure coordinated care with other service providers. A bachelor's degree is required, and candidates must be at least 21 years old with a valid AZ driver’s license.
