Behavioral Health Case Manager

The Behavioral Health Case Manager (BHCM) serves as a point of contact and advocate to members. They provide face‑to‑face, phone, virtual, and in‑home services as needed by each member's journey to recovery. They develop, implement, and update person‑centered individual service plans (promoting member participation) in a timely manner. They monitor progress on plans, goals, objectives, and status on an ongoing basis with the person served, documenting the progress on the service plan as defined by policies and procedures. They serve as a point of contact and advocate as needed to ensure ongoing collaboration, including the communication of appropriate clinical information regarding progress of the person served with other individuals and/or entities with whom delivery and coordination of covered services is important (e.g., primary care providers, adult probation, other involved service providers). Minimum Qualifications Bachelors or master’s degree, no experience necessary. Associate degree plus one (1) year* behavioral health work experience. High School Diploma or GED plus Peer Support Specialist certification plus two (2) years* of behavioral health work experience. High School Diploma or GED plus Medical Assistant certification plus two (2) years* of behavioral health work experience. High School Diploma or GED plus three (3) years* of behavioral health work experience. Qualified Applicants Must be able to pass a pre‑employment drug screen and background check. Unacceptable driving record includes (but is not limited to) the following: more than two [2] moving violations in the last three [3] years more than one [1] at‑fault accident within the last three [3] years any major violation (i.e., DUI, Careless/Reckless Driving, Drag Racing, Hit and Run) within the last three [3] years At time of job offer, submission of 36 month (three year) driving record from DMV will be required.
