Source of Hope LLC is a dynamic and growing organization dedicated to making a positive impact in our community. We are seeking a highly motivated and organized Program Coordinator to join our team and help us achieve our mission.

 

Key Responsibilities

• Request current Person-Centered Service Plans (PCSPs) and verify that required PCSPs are received, current, and maintained in each member's file.
• Ensure each member's authorized goals are accurately entered into SpokeChoice and communicate/explain the goals to assigned providers so they understand the supports and activities expected under the PCSP.
• Monitor authorized service-hour utilization. Train, assist, and remind providers who are not using their available hours appropriately so authorized services are not unnecessarily left unused.
• Verify that guardians review and approve provider service hours on time; promptly follow up with guardians when approvals are pending or missed.
• Coordinate, complete, schedule, and/or assign required home visits and PCSP meetings, and ensure related documentation is completed and filed.
• Provide operational and administrative assistance to the Program Manager on assigned and as-needed tasks.
• Assist with provider onboarding, including coordination, documentation follow-up, system setup support, and communication as assigned.
• Provide oversight and follow-up for Case Manager task completion, including home visits, PCSP-related responsibilities, required documentation, and other assigned duties. Escalate incomplete or overdue items to the Program Manager.
• Maintain accurate, timely, and confidential records in company systems and member/provider files.
• Complete other reasonable duties and special projects as assigned by management.

Qualifications:

- Bachelor's degree in a related field or equivalent work experience.

- Fluent in English.

- Proven experience in program coordination or a related role.

- Strong organizational and project management skills.

- Excellent communication and interpersonal abilities.

- Proficiency in Microsoft Office, Excel, and project management tools.

- Ability to work independently and as part of a team.

- Strong problem-solving and decision-making skills.
