The Community – Chinle, AZ The Chinle Comprehensive Health Care Facility (CCHCF) is based in Chinle, Arizona (Northeast Arizona near Canyon De Chelly National Monument). Strong Navajo cultural traditions exist within the community, offering an opportunity to learn the Navajo language or learn about traditional Navajo medicine. The service unit is located on the Colorado Plateau with excellent opportunities for photography, hiking, running, road biking, cross-country skiing, and mountain biking. Canyon de Chelly National Monument is within 5 miles of the hospital and is a wonderful place for exploring and sightseeing, running, and mountain biking. About the Facility – Chinle Comprehensive Health Care Facility (CCHCF) The CCHCF provides medical care for approximately 35,000 Navajo citizens. Serving a rural area, many of the patients travel 100 miles round trip to receive care and do not have electricity or running water in their homes. In addition, most of the elderly patients speak only Navajo and live according to traditional Navajo cultural practices. The CCHCF and Ambulatory Care Center is a 60-bed inpatient hospital and outpatient facility. Services offered include: Adult Inpatient, Labor & Delivery, Pharmacy, Adult Intensive Care, Mental Health, Physical Therapy, Anesthesiology, Nurse Phone Triage Services*, Podiatry, Audiology, OB/GYN & Midwifery, Public Health, Dental, Occupational Therapy, Radiology, Emergency Medicine, Optometry, School Health, General Surgery, Outpatient Primary Care, Speech Pathology, Lab, Pediatric Inpatient, Women's Health.

Pharmacist - Responsibilities (listed are included but not limited to)

• Perform pharmacy or direct patient care services.
• Adhere to competency standards for outpatient pharmacy services at CCHCF.
• The Pharmacist documents patient care using CCHCF's electronic health record system.
• Manage medication therapy, evaluate patient responses, make recommendations, and monitor care plans.
• Gather patient health history, adjust treatment plans, and update clinical activities, problem lists, and medication allergy information in the EHR.
• The Pharmacist evaluates medication regimens for appropriateness, interactions, compliance, and adverse events.
• Accurately dispense prescriptions, counsel patients on medication use, storage, and side effects, and document provided cognitive services.
• Adhere to safety reporting requirements outlined in CCHCF's Safety Measure Policy.
• Rotate on-call shifts, provide drug information, process inpatient orders, prepare IVs, and restock medication dispensers.
• Serve patients of all ages in outpatient and inpatient settings.
• The Pharmacist reviews electronic health records to ensure appropriate prescriptions, verify accuracy, and counsel patients on proper medication use, dosage, and expected outcomes.

Pharmacist - Requirements

• Must be board certified.
• Must be a graduate from a Pharmacist Educational program, accredited by the Accreditation Council for Pharmacy Education or its predecessors, at a college, university, or educational institution that is accredited by an accrediting body or organization recognized by the U.S. Department of Education at the time the degree was obtained.
• Shall possess a current, valid, and unrestricted pharmacy license in a state, the District of the Commonwealth of Puerto Rico, or a Territory of the United States.
• Shall have current BLS, ACLS, and/or PALS preferred.
