Job Description:

* Regular hours of 8am-5pm, Monday through Friday, including home visits
* Rotating on-call schedule - a few nights or weekends per month

On the day-to-day, you will:
* Provide assessments, planning, primary care, and continuous medical coverage to participants
* Collaborate with community-based physicians and the IDT in discharge planning from acute or long-term settings
* Actively participate in IDT meetings, communicating participant changes, collaborating on care planning decisions, and coordinating 24-hour care delivery
* Help maintain participant care in nursing homes, making regular visits as dictated by participant need

Job requirements include:
* Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
* Master’s degree or doctorate degree
* Board certification - internal medicine, family practice, or emergency medicine; geriatrics preferred.
* Active State Physician License without restrictions, as well as state CDS certification and DEA registration
* Must be willing and able to participate in rotating night and weekend call
