Family Practice/Primary Care Nurse Practitioner

We are seeking a Primary Care Nurse Practitioner to deliver exceptional care in a collaborative, team-based environment.

Responsibilities

• Provide primary care services in a 100% outpatient clinic setting, working alongside one other provider and support staff.
• Maintain a Monday-Friday schedule, 8am-5pm, with no late nights or weekends.
• Focus on primary care for the adult population, excluding pediatrics and women's health.
• Manage call duties via phone only, with a rotated schedule.
• Aim to treat 22-25 patients per day, adjusting for no-show rates as needed.
• Operate independently in a lean clinic environment.
• Collaborate with a team offering a full scope of services, including dental, behavioral health, and social work.

Requirements

• Possess an active or applied NY State license, DEA, and CSR.
• Hold training as a Nurse Practitioner in Family or Adult care.
• Preferably have 5 years of experience as a Nurse Practitioner.
