Care Coordinator

Company Description Senior Services of Will County Inc. connects older adults and their families with resources, information, services, and programs throughout Will County. The organization focuses on helping seniors remain safe, healthy, and independent in their homes and communities. Role Description This full-time Care Coordinator role is a hybrid position based in Romeoville, IL, with the flexibility to perform some duties from home. The Care Coordinator will assess client needs, develop individualized care plans, and connect clients with appropriate community resources, programs, and services. Daily responsibilities include conducting in-home assessments, coordinating home care and supportive services, and monitoring service delivery to ensure client safety, health, and independence. The role also involves maintaining accurate case records, documenting all contacts and interventions, and collaborating with internal team members and external providers. The Care Coordinator will respond to client and family inquiries, provide education on available supports, and advocate for clients to ensure timely access to assistance. Hours: M-F 8:00 to 4:30 OR 7:30 to 4:00 Qualifications Demonstrated experience in Care Coordination and Case Management, including assessment, service planning, and follow-up. Strong Communication and Customer Service skills, with the ability to build rapport and work effectively with diverse older adult populations and their support networks. Knowledge of Home Care services, community-based resources, and aging-related programs, or willingness to learn these areas quickly. Ability to manage a caseload, prioritize tasks, and maintain detailed documentation in electronic and paper records. Bachelors degree in social work, human services, gerontology, or a related field preferred. Years of experience can be considered in lieu of degree. Ability to maintain a monthly quota. Comfortability in conducting assessments in clients’ homes. Proficiency with basic computer applications (e.g., email, electronic case management systems, word processing, and spreadsheets). Valid driver’s license, reliable transportation, and ability to travel locally within Will County for client visits.
