Through our 12-nursing home, 10 assisted living, and 8 independent living communities encompassing nearly 2,000 beds, we provide exceptional health care services, including long-term nursing care, assisted and independent living, memory care, hospice, home care, respite care, and short-term post-acute rehabilitation.

Qualifications: 2 years of experience in long term care, licensed social worker or bachelor's degree preferred, must possess excellent communication and organizational skills

Essential Job Functions:

• Develop one-to-one relationships with residents and families.
• Establish relationships with area hospice agencies.
• Act as a liaison with social, health and community agencies.
• Coordinate with Activity Department to meet socialization needs of the residents.
• Oversee coordination of referrals for ancillary services (dentist, eye doctor, podiatry, psychology, psychiatry, and audiology).
• Assist in admissions and discharge process as needed.
