Clinical Pharmacist The Veterans Health Administration is seeking Clinical Pharmacists for the Tomah VA Medical Center in Tomah, WI. This position functions within clinical sections of the Pharmacy service line under the Chief of Staff's Office. The Clinical Pharmacist interprets and verifies medication orders, reviews patient medication profiles, provides drug and policy information to patients and medical center staff, supports formulary maintenance, participates in interdisciplinary care, and contributes to pharmacy education and quality improvement programs. Location: Tomah, WI Vacancies: Many vacancies Salary: $140,195 - $182,257 per year Pay Scale & Grade: GS-12 Promotion Potential: None Work Schedule: Full-time, Monday - Friday, 8:00am - 4:30pm Additional Schedule Details: Regular rotation staffing in the Inpatient Pharmacy on Sundays, 7:00am - 11:00am, and holidays, 7:00am - 3:30pm. Typical frequency is approximately one Sunday shift every two months and one holiday shift per calendar year. EHR Implementation Schedule Note: Tomah VA is implementing a new health record in January 2027. Post go-live staff may be required to work alternative shifts, including evenings, nights, and weekends, until implementation is complete. Consult with medical center staff on therapeutic or policy questions related to outpatient prescriptions or inpatient physician orders. Ensure formulary guidelines are followed according to applicable rules, regulations, and guidelines. Ensure accuracy of drug dispensing based on patient history, medication profile review, good clinical practice, and other relevant factors. Provide drug information to physicians, nurses, and other health care professionals. Provide oral and written drug information to inpatient and outpatient Veterans as appropriate. Conduct clinical screening procedures, including creatinine clearance calculations for at-risk patients. Provide discharge medication counseling to Veterans and families discharging home from medicine service and short-stay rehabilitation. Assist the Chief of Pharmacy with formulary maintenance. Gather information supporting inclusion or removal of drugs from formulary at the national, VISN, or local level. Participate in interdisciplinary rounds, including discharge planning and falls assessments. Make recommendations to improve therapy and support transition to home. Assist with instruction of professional or technical pharmacy students. Assist in training new technicians and pharmacists. Participate in quality improvement activities involving drug therapy and clinical pharmacy functions. Provide a monitoring program for adverse drug reactions as required by The Joint Commission. Education and Licensure Requirements: Must be a graduate of an Accreditation Council for Pharmacy Education accredited College or School of Pharmacy with a baccalaureate degree in pharmacy or a Doctor of Pharmacy degree. Foreign pharmacy graduates meet the education requirement if they provide proof of Foreign Pharmacy Graduate Examination Commission certification, including passing the FPGEE and TOEFL iBT. Must have a full, current, and unrestricted license to practice pharmacy in a U.S. State, Territory, Commonwealth, or the District of Columbia. Must maintain current registration if required for maintaining full, current, and unrestricted licensure. GS-12 Qualification Requirements: Must meet the basic pharmacist education, licensure, and English language proficiency requirements. Must have one full year of creditable experience at the next lower level, GS-11 or equivalent, directly related to Clinical Pharmacy work; or Must have completed an ACPE-accredited Pharm.D. program. Qualifying experience includes reviewing, interpreting, and verifying medication orders for appropriateness. Qualifying experience includes processing and filling medication orders. Qualifying experience includes interacting with clinical staff and making medication therapy recommendations to ensure safe and effective care. Qualifying experience includes reviewing patient medications, allergies, labs, and medical record information to identify and solve medication-related problems. Qualifying experience includes contacting providers, documenting recommendations and interventions, and providing refill extensions or partial medication supplies. Qualifying experience includes taking health and medication histories and performing medication reconciliation. Qualifying experience includes providing drug information and assisting with formulary management, therapeutic substitutions, nonformulary reviews, medication usage evaluations, and adverse drug event documentation. Qualifying experience includes providing oversight of technical staff in medication distribution. Required Knowledge, Skills, and Abilities: Knowledge of professional pharmacy practice. Ability to communicate orally and in writing with patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing drug therapy outcomes, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Experience working in an outpatient setting or health system pharmacy as an intern, resident, or pharmacist. Education: Transcripts are required if using education to qualify. Education must be from an accredited institution recognized by the U.S. Department of Education. Foreign education must be evaluated as equivalent to U.S. education by a recognized credential evaluation organization.
