Medical, dental, and vision insurance 401(k) retirement plan Paid time off Paid holidays Opportunities for professional growth and advancement Supportive, team-oriented work environment Job Details The MDS Coordinator will oversee and coordinate the completion of the Minimum Data Set (MDS) assessment process while collaborating closely with nursing, therapy, social services, dietary, and other members of the interdisciplinary care team. Responsibilities: Coordinate and complete MDS assessments accurately and within required timeframes Manage the RAI/MDS process in accordance with CMS and state regulations Develop and maintain individualized resident care plans in collaboration with the interdisciplinary team Monitor assessment schedules and ensure timely completion of required documentation Review clinical documentation for accuracy, completeness, and appropriate reimbursement Support Medicare and Medicaid reimbursement processes, including PDPM Participate in care plan meetings and communicate with residents, families, and members of the care team Monitor changes in resident condition and ensure assessments and care plans are updated appropriately Maintain compliance with federal, state, and facility standards Assist with audits, surveys, and quality improvement initiatives Serve as a resource to clinical staff regarding MDS documentation and regulatory requirements Qualifications: Active nursing license or ability to obtain one Previous MDS experience in a skilled nursing or long-term care setting preferred Strong understanding of the RAI process, MDS 3.0, Medicare/Medicaid guidelines, and PDPM Strong organizational skills and attention to detail Ability to manage deadlines and multiple assessments simultaneously Excellent communication and interdisciplinary collaboration skills Knowledge of long-term care regulations and clinical documentation requirements Jobot is an Equal Opportunity Employer.
