Responsibilities
- Perform initial PT evaluations and develop individualized treatment plans for residents in a skilled nursing setting
- Deliver evidence-based therapeutic interventions to improve mobility, strength, balance, gait, and functional independence
- Reassess resident progress, modify treatment plans as needed, and document outcomes in accordance with policy
- Collaborate with an interdisciplinary team (nurses, physicians, therapy aides, and case managers) to coordinate care
- Educate residents and families on goals, progress, safety, and home exercise programs; participate in discharge planning as appropriate
- Ensure patient and staff safety, maintain infection control standards, and adhere to regulatory and documentation requirements

Requirements
- Active Physical Therapist license in Mississippi or eligibility to practice in the assignment state
- Doctor of Physical Therapy (DPT) degree or equivalent
- Experience in physical therapy with a preference for skilled nursing facility or long-term care settings
- Strong communication and collaboration skills; ability to work as part of an interdisciplinary team
- Ability to work within a skilled nursing facility environment and support a travel assignment
