Job Summary

Fletcher Rehabilitation and Healthcare Center, a Long Term Care facility located in Fletcher, NC, is seeking a highly skilled and compassionate MDS Coordinator to join our team. As an essential member of our care team, you will play a vital role in developing comprehensive care plans for our residents, ensuring timely and effective care, and maintaining accurate documentation. Essential Functions

• Keep abreast of current federal and state regulations, as well as professional standards.

• Assist with the development of comprehensive care plans for Residents in coordination with the MDS that accurately addresses the needs of the Resident.

• Coordinate, manage, and monitor the written plan of care for each Resident of the facility that identifies the needs of the Resident and goals to be accomplished for each need.

• Assist nursing management with the coordination, management, and review of nurse’s notes to determine if the care plan is being followed.

• Monitor Resident status changes to ensure appropriate and timely nursing or clinical team involvement.

• Ensure MDS and support documentation are accurate representations of the Resident and meet regulatory and auditor requirements.

• Perform regular audits of documentation to assure accuracy.

• Assist nursing management with the discharge process.

• Perform administrative requirements, such as completing necessary forms and reports.

• Ensure that established infection control and standard precaution practices are maintained when providing care.

• Maintain strict confidentiality regarding sensitive health information of Residents.

• Report all hazardous conditions, damaged equipment and supply issues to appropriate persons.

• Maintain the comfort, privacy, and dignity of Residents and interact with them in a manner that displays warmth, respect, and promotes a caring environment.

• Answer and respond to call lights promptly and courteously when working in Resident care areas.

• Communicate and interact effectively and tactfully with Residents, visitors, families, peers, and supervisors.

• Attend and participate in departmental meetings and in-services as directed.

Required Education and Experience

• Current State License as a Nurse

• C.P.R. Certified

• MDS experience

Preferred Education and Experience

• One year experience as a Nurse in a long-term care setting.

• Knowledge and training in all aspects of MDS process.
