Hospice Home Care RN Case Manager

Gilchrist Hospice Inova is seeking an experienced RN Case Manager to provide expert nursing and supportive care to terminally ill patients and their families in the Northern Virginia region. This role reports to the Team Manager and requires collaboration with the interdisciplinary team to identify needs and deliver comprehensive home or facility-based hospice care. Ideal candidates hold a Diploma/ADN/BSN, Virginia RN licensure, and at least two years of nursing experience with strong #J-18808-Ljbffr Gilchrist
