Intake Specialist (LPN)In-office preferred | Remote consideredWe are seeking an experienced Licensed Practical Nurse (LPN) to join our team as an Intake Specialist. This role is critical to ensure timely, accurate admissions and strong communication between referral sources, patients, and clinical staff.Position SummaryThe Intake Specialist (LPN) manages the intake and referral process from initial referral through Start of Care. This position requires strong clinical knowledge, attention to detail, and the ability to coordinate efficiently in a fast-paced home health environment. While local in-office candidates are preferred, remote candidates will be considered based on experience and qualifications.Key ResponsibilitiesReceive, review, and clinically screen referrals from hospitals, physicians, facilities, and community partnersVerify patient eligibility, insurance coverage, and required documentationCoordinate and schedule Start of Care (SOC) visits with clinical staffCommunicate professionally with referral sources, patients, families, and internal team membersAccurately enter and maintain patient information in the EMRTrack referral status and follow up to ensure timely admissionsEnsure compliance with Medicare, Medicaid, and agency intake requirementsCollaborate with clinical, billing, and administrative teams to support continuity of careRequired QualificationsActive LPN license (required)Home health experience strongly preferredIntake, admissions, or care coordination experience preferredKnowledge of Medicare, Medicaid, and insurance authorization processesStrong organizational and time-management skillsExcellent verbal and written communication skillsAbility to manage multiple referrals simultaneouslyProficiency with EMR systems and Microsoft OfficeWork LocationIn-office position preferred (Indianapolis area)Remote options may be considered for highly qualified candidates with relevant home health intake experienceWhat We OfferCompetitive pay based on experienceSupportive, team-oriented work environmentOpportunity to grow within a respected home health organizationMeaningful work impacting patient care and outcomesHow to ApplyInterested candidates should submit their resume for consideration.Caregivers, Inc. is an Equal Opportunity Employer.
