Key Responsibilities

• Perform patient evaluations by obtaining history, conducting systems reviews, and administering tests and measures to collect clinical data
• Establish individualized plans of care and select treatment techniques that address identified impairments in accordance with physician orders
• Conduct ongoing reassessment of patients and their response to treatment, initiating and managing discharge planning from the first visit
• Supervise and delegate to ancillary staff and students consistent with state laws and department policy
• Complete accurate, legible documentation within established timeframes and in accordance with accreditation and insurance requirements
• Communicate with patients, families, caregivers, and the broader healthcare team to promote maximum benefit of care

Required Qualifications

• Minimum of 2 years of Physical Therapist experience in a Long Term Care setting
• Active Texas PT license or compact license required
• Must function within scope of practice as defined by the Texas state practice act

Preferred Experience / Skills

• Experience with quality improvement initiatives in a clinical setting
• Demonstrated ability to assess and address educational needs of patients, families, and caregivers
• Participation in professional development activities such as workshops, in-services, and continuing education
