The ILST works with each client on recovering skills that have decreased because of onset disabilities of progressive illness to maintain essential skills within the clients home and community. ADLs and IADLs- Self-care training medication management task completion, communication and interpersonal skills, Home organization Mail Coordination Meal preparation providing transportation to medical appointments, social and community outings community transportation skills, finance management, pre-vocational skills Company Description Amor Coordination Services, Inc. is a company dedicated to assisting Medicaid Waiver participants on the Nursing Home Transition Diversion and the Traumatic Brain Injury Programs stay within their homes and community.
