• Manage and oversee a designated home health patient caseload from an office setting
• Coordinate interdisciplinary patient care with registered nurses, therapists, social workers, and physicians
• Review and approve clinical documentation for accuracy, completeness, and regulatory compliance
• Communicate with the field clinicians/DON to decide the initial plan of care based on initial patient assessment and physician’s orders
• Monitor patient outcomes and identify opportunities to improve quality of care
• Participate in clinical outcomes monitoring, follow up and agency performance improvement initiatives
• Provide clinical guidance and support to field clinicians
• Address clinical concerns, patient complaints, and care coordination issues in a timely manner
• Gain quality experience in working with a multidisciplinary team and personal patient interaction
• Participate in quality assurance and performance improvement (QAPI) activities
