Job Title: Case Management Assistant
Schedule: On site | 7:30 AM - 3:30 PM PST | Rotating weekends, holidays required


Requirements:


High School Diploma or equivalent experience
1+ years of recent relevant experience in care coordination, case management support, or discharge planning
Experience with Epic EHR (required)
Experience with utilization review (UR) transmission
Experience with high-volume fax and voicemail management
Experience with data entry in a healthcare setting
Ability to work PST hours regardless of location
Ability to work rotating weekends and required holidays
Ability to work independently in a fast-paced environment with shifting priorities


Preferred Requirements:


Experience supporting Transition of Care (TOC) or discharge planning workflows
Experience coordinating referrals, authorizations, and post-acute placements
Experience with Allscripts or similar EMR systems
Proficiency in documenting care coordination notes in EMR systems
Strong written and verbal communication skills
Strong time management and prioritization skills
Ability to collaborate with Case Managers and Social Workers


Summary / Duties:
Supports case management and Transition of Care (TOC) discharge planning for ambulatory patients by coordinating activities across the care continuum to ensure timely discharges. Partners with Case Managers and Social Workers to assess patient needs, communicate care plans, and coordinate post-discharge services, referrals, and authorizations. Manages high-volume administrative tasks including fax, voicemail, and data entry while maintaining accurate documentation in Epic and other EMR systems. Facilitates patient transfers to post-acute facilities, prepares required documentation, and communicates updates to internal and external stakeholders. Participates in daily huddles, escalates barriers to discharge, and ensures compliance with HIPAA and departmental standards.
