Home Health Physical Therapist - PRN
Location: Pearland, Friendswood, Pasadena, Webster, Alvin, Missouri City, Brazoria and Galveson Counties

About the Role:

As a Physical Therapist, you will play a critical role in helping patients recover from injuries, manage chronic conditions, and improve their overall physical function and mobility. You will assess patients' physical abilities, develop personalized treatment plans, and implement therapeutic interventions to promote healing and prevent further injury. Your work will involve collaborating with healthcare professionals to ensure comprehensive care and support for patients across various age groups, including geriatric populations. You will also educate patients and their families on exercises, injury prevention, and lifestyle modifications to enhance long-term health outcomes. Ultimately, your efforts will contribute to improving patients' quality of life by restoring their independence and physical well-being.

Minimum Qualifications:

• Doctor of Physical Therapy (DPT) degree from an accredited program.
• Valid state license to practice as a Physical Therapist in the United States.
• Strong knowledge of physical therapy principles, orthopedic rehabilitation, and manual therapy techniques.
• Experience in assessing and treating musculoskeletal and sports-related injuries.
• Excellent communication and interpersonal skills to effectively interact with patients and healthcare teams.

Preferred Qualifications:

• Certification in Orthopedic Clinical Specialist (OCS) or Sports Physical Therapy (SCS).
• Experience working with geriatric populations and managing age-related rehabilitation challenges.
• Familiarity with advanced rehabilitation technologies and modalities.
• Background in sports medicine and injury prevention programs.
• Additional training or certification in manual therapy techniques.

Responsibilities:

• Conduct thorough patient evaluations to assess physical conditions, limitations, and rehabilitation needs.
• Develop and implement individualized treatment plans using techniques such as manual therapy, orthopedic rehabilitation, and exercise programs.
• Monitor patient progress and adjust treatment plans accordingly to achieve optimal recovery outcomes.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care and ensure holistic treatment.
• Educate patients and caregivers on injury prevention, therapeutic exercises, and strategies to maintain physical health.
