As Part Of Our Nursing Team You Will:

• Oversee patient care services
• Assess new referrals
• Visit clients at home to evaluate for services
• Develop care plans and introduce home care aides to new clients
• Supervise and support our home health aide staff in the field
• Provide medication management for clients
• Help ensure that medication is taken safely and as directed.
• Develop and implement a Plan of Care related to medication, includes calling in refills and contacting the MD with questions as needed.

Work Schedule / Coverage Area

Weekdays: 2-4 days/week

Visit clients in the Metro North, Merrimack Valley, North Shore and Cape Ann communities

Requirements

• A current Massachusetts RN/LPN license with at least 2 years nursing experience
• Strong clinical and computer skills
• Excellent communication skills
• A team player that is self-motivated with the ability to function autonomously.
• The ability to prioritize and demonstrate problem solving skills.
• Previous home care experience is preferred; Medicare/Certified service experience is not required.
