Nursing Team Opportunity

As Part Of Our Nursing Team You Will:
- Oversee patient care services
- Assess new referrals
- Visit clients at home to evaluate for services
- Develop care plans and introduce home care aides to new clients
- Supervise and support our home health aide staff in the field
- Provide medication management for clients
- Help ensure that medication is taken safely and as directed.
- Develop and implement a Plan of Care related to medication, includes calling in refills and contacting the MD with questions as needed.
Work Schedule / Coverage Area
Weekdays: 2-4 days/week
Visit clients in the Metro North, Merrimack Valley, North Shore and Cape Ann communities
Requirements
- A current Massachusetts RN/LPN license with at least 2 years nursing experience
- Strong clinical and computer skills
- Excellent communication skills
- A team player that is self-motivated with the ability to function autonomously.
- The ability to prioritize and demonstrate problem solving skills.
- Previous home care experience is preferred; Medicare/Certified service experience is not required.
