Responsibilities: Obtain health histories and document health concerns of patients Serve as a liaison to needed services and follow up on problems or concerns presented by the resident Suggest appropriate community resources to meet specific needs and enhance the client’s well-being Educate patients/caregivers about their diagnoses, medications and help them become independent with their treatments and self-management of their chronic illness(es) What you will need: Required: A current license to practice as a Registered Nurse (RN) in the State of New York 2 years or more of recent experience as a RN ideally in home care, ED, ICU or oncology Valid driver license and reliable vehicle Preferred: 2 years or more of experience in a home care setting Bachelor's degree of nursing Registered Nurses at Eddy VNRA must possess a passion for caring for patients and demonstrate a respectful, caring, professional demeanor The RN must have excellent time management and organizational skills The RN must be able to communicate effectively, verbally and written and possess strong critical thinking skills to problem solve Pay Range: $40.00 - $52.81 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
