As a Household Coordinator with Heritage House, your responsibilities will include:

• Set an example of encouraging resident choice and providing a social model of care.
• Provide leadership as a member of the Nursing Leadership Team
• Perform all of the duties of a nurse for the purpose of providing health care services to residents and their families.
• Assist in the evaluation of the health care services needed for each resident relative to the level of lifestyle accommodation.
• Participate in ensuring the community is following all applicable regulations at all times.
• Maintain confidential relations with residents and families while assisting in providing spiritual, psychological, and social needs of residents
• Participates in on-going Quality Assurance programming and conducts audits as requested
• Assists in documentation for MDS data collection

Qualifications:

• Currently licensed LPN in the State of Iowa
• Prior clinical management experience required, prior long term care experience strongly preferred
