In this role, you will evaluate and treat residents, develop individualized therapy plans, and collaborate with physicians, families, and interdisciplinary team members to maximize resident mobility, function, and independence.Key ResponsibilitiesEvaluate residents and develop individualized treatment plansProvide skilled physical therapy services in accordance with physician orders and established goalsAssess functional abilities, mobility, strength, balance, and enduranceMonitor resident progress and modify treatment plans as appropriateCommunicate effectively with residents, families, physicians, nursing staff, and interdisciplinary team membersComplete accurate and timely documentation in compliance with facility, state, federal, and payer requirementsParticipate in care planning meetings and discharge planning activitiesEducate residents, families, and caregivers on treatment plans, safety techniques, and home exercise programsConduct in-service education and training for facility staff as neededRecommend adaptive equipment and interventions to improve safety and independenceMaintain compliance with all facility policies, state practice acts, and regulatory requirementsSupervisory ResponsibilitiesSupervise Physical Therapist Assistants (PTAs), therapy aides, and students in accordance with state regulations and facility policiesQualificationsEducation & ExperienceBachelor's degree in Physical Therapy requiredMaster's or Doctorate degree in Physical Therapy preferredExperience in skilled nursing, rehabilitation, or long-term care preferredProficiency with Net Health and PointClickCare (PCC) preferredStrong clinical, communication, and documentation skillsLicenses & CertificationsCurrent Physical Therapist license in South Carolina and in good standingMust maintain all required continuing education requirementsMust remain in good standing with the applicable State BoardWe are an equal opportunity employer.
