We are seeking a compassionate and dependable Part-Time Hospice Aide/Certified Nursing Assistant (CNA) to provide exceptional personal care to hospice patients in the Dallas area.

Position Summary
The Hospice Aide/CNA provides hands-on personal care and assistance with activities of daily living to hospice patients in their homes, assisted living communities, nursing facilities, or other places of residence.

Working under the supervision of a Registered Nurse and following each patient’s individualized Plan of Care, the Hospice Aide helps maintain the patient’s comfort, dignity, safety, and quality of life.

Position Detail

• Part-time position

• Primary service area: Dallas
• Local travel between patient residences is required
• Schedule and visit assignments are based on patient and agency needs

Key Responsibilities

Personal Care and Assistance

• Provide direct personal care according to the patient’s individualized Plan of Care.
• Assist patients with bathing, grooming, oral hygiene, skin care, and other personal hygiene needs.
• Assist with dressing and toileting.
• Help patients with activities of daily living.
• Provide mobility assistance, transfers, repositioning, and other approved supportive care.
• Provide feeding assistance when included in the Plan of Care and permitted by agency policy.
• Maintain a clean, safe, and comfortable patient environment.
• Use proper body mechanics and follow established safety procedures during all patient-care activities.
• Protect each patient’s privacy, dignity, preferences, and personal choices.

Observation and Communication

• Observe patients for changes in physical condition, behavior, comfort, skin integrity, mobility, and ability to complete daily activities.
• Promptly report changes in condition, new symptoms, safety concerns, or caregiver concerns to the RN Case Manager.
• Report any refusal of care, missed visit, or inability to complete assigned services.
• Communicate professionally and compassionately with patients, families, caregivers, facility staff, and members of the hospice team.

Care Coordination

• Follow the established Plan of Care under the direction and supervision of the Registered Nurse.
• Complete assigned visits according to the established schedule.
• Collaborate with members of the interdisciplinary hospice team, including:
  • RN Case Managers
  • Social workers
  • Chaplains and spiritual-care providers
  • Physicians and clinical leadership
  • Other members of the patient’s care team
• Notify the supervising nurse when the patient’s needs appear to require an update to the Plan of Care.
• Participate in care coordination activities and team meetings when required.

Patient and Family Support

• Provide a calm, compassionate, and respectful presence for patients and families.
• Reinforce basic personal-care instructions and comfort measures as directed by the Registered Nurse and Plan of Care.
• Listen respectfully to patient and family concerns and communicate those concerns to the RN Case Manager.
• Maintain appropriate professional boundaries at all times.

Documentation and Compliance

• Complete all Hospice Aide/CNA visit notes accurately and within the required 24–48-hour timeframe.
• Maintain complete and professional documentation in the agency’s electronic medical record system.
• Document the care provided, patient response, observations, and concerns requiring follow-up.
• Follow all hospice policies, procedures, infection-control requirements, safety practices, and professional standards.
• Protect patient confidentiality and comply with HIPAA requirements.
• Perform only those duties permitted by the CNA’s certification, the patient’s Plan of Care, and agency policy.

Required Qualifications

• Active Certified Nursing Assistant certification in the State of Texas.
• Three to five years of CNA experience.
• Valid driver’s license and reliable transportation.
• Ability and willingness to travel throughout the assigned Dallas service area.
• Ability to work independently while collaborating with an interdisciplinary hospice team.
• Strong communication, organization, and time-management skills.
• Dependability and the ability to complete assigned visits and documentation on time.
• Compassionate, respectful, and patient-centered approach to care.
• Ability to successfully complete required background checks, certification verification, and onboarding requirements.
• Commitment to maintaining confidentiality and complying with HIPAA and agency policies.

Preferred Qualifications

• Previous hospice or home health experience.
• Experience using an electronic medical record system, such as Kinnser/WellSky.
• Understanding of hospice philosophy, end-of-life care, and comfort-focused services.
