Home Health Occupational Therapist OT

Job Summary:

PathWell Health, a trusted and community-focused home health care provider, seeks a compassionate and experienced Home Health Occupational Therapist OT to join our team in San Diego, California . As an integral member of our health care team, you will have the opportunity to make a meaningful difference in the lives of individuals with disabilities or developmental delays.

Responsibilities:

• Develop, implement, and evaluate plans of care in partnership with clients, representatives, caregivers, and physicians

• Provide direct occupational therapy services in accordance with established plans of care

• Educate and instruct clients, family members, or other representatives on rehabilitation care and activities to promote health, safety, and independent living

• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided

• Select activities that promote work and life-management skills within clients' mental or physical capabilities

• Recommend changes to patients' work or living environments to ensure their needs and capabilities are met

• Engage in research activities and participate in formal educational training related to therapy

Minimum Qualifications:

• Current and valid California Occupational Therapist license

• Recent work experience in home health (preferred)

• Driver's License, Car Insurance, Car Registration

• Strong clinical assessment, organizational, and communication skills

• Ability to handle multiple assignments simultaneously

• Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing, and documentation

• Bilingual (Spanish) preferred

• Excellent oral and written communication skills

• Excellent customer relationship skills

Compensation:

• Salary: $90,000 - $120,000 annually
