Registered Nurse- PACT Care Manager

Summary This position is eligible for the Education Debt Reduction Program (EDRP) – a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program approval determines the award amount (up to $200,000) and eligibility period (one to five years) by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants are ineligible to apply. Qualifications Basic Requirements: English language proficiency – proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency (ACEN or CCNE) or attending a master's level bridge program in nursing with coursework equivalent to a bachelor's degree in nursing. Upon achievement of a state license, the individual may be appointed on a temporary basis and later converted to a permanent appointment after successful completion and graduation from the bridge program. (Reference VA Handbook 5005 – Appendix G6) Foreign‑school graduates must possess a current, full, active, and unrestricted registration to meet the requirement for graduation from an approved professional nursing school. Credit for foreign nursing education higher than an associate degree requires a formal equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD). Graduate nurse technician (GNT) exception: candidates who otherwise meet the basic education requirements but do not possess the required licensure may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120‑day temporary appointment while actively pursuing licensure (may be extended up to two years on a case‑by‑case basis). Grade determinations are based on scope, education, and dimension criteria. Detailed requirements for each grade level are available in the online assessment portal. Preferred experience: BSN, 2 years of RN experience, 1–2 years of primary care experience, experience managing primary care panels, and experienced in case management. Physical requirements: heavy lifting (35 lbs and over), heavy carrying (35 lbs and over), straight pulling (up to 1 hour), pushing (1–2 hours), reaching above shoulder, use of fingers, both hands required, walking (up to 4 hours), standing (up to 4 hours), kneeling (up to 2 hours), repeated bending (up to 4 hours), both legs required, rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16", far vision correctable to 20/20 in one eye and 20/40 in the other, ability to distinguish basic and shades of colors, hearing (aid permitted), and emotional/mental stability. Duties The Staff Registered Nurse (RN) is responsible for providing competent, evidence‑based outpatient care to patients and overseeing licensed vocational nurses/nursing assistants as appropriate. Assumes responsibility for coordinating care focused on patient transition through the continuum of care, patient and family education, patient self‑management after discharge, and supporting factors that impact customer satisfaction. Assists in directing the provision of nursing education, orientation, competencies, and quality improvement to enhance outcomes utilization. Integrates outpatient knowledge, skills, abilities, and judgment; is self‑directed in goal setting for managing complex situations. Validates understanding of the rationale for care processes and decisions; questions clinical practices for the purpose of providing evidence‑based care. Demonstrates the ability to cope with and manage competing priorities; exhibits competence in outpatient practice and decision making with deliberate planning and critical thinking skills. Independently plans and delivers technically competent care to patients with complex needs in alignment with ANA nursing standards. Manages patient care organization and prioritization, ensuring assigned tasks are completed. Functions as an emerging leader in providing direction to staff to identify, implement, and evaluate patient care needs. Collaborates with the interdisciplinary team to ensure the plan of care is achieved. Provides and directs veteran/patient‑driven care that optimizes patient experience, satisfaction, and safety for a team of patients. Maintains professional boundaries to protect patient vulnerabilities and act in the best interest of the patient. Models ethical behavior, high reliability principles, and I CARE values to improve patient outcomes. Demonstrates knowledge across a wide range of job responsibilities, effectively communicating with individuals and groups to build teamwork and purpose. Actively contributes to the collaboration process and clearly communicates ideas to promote team success. Makes meaningful and challenging contributions for the betterment of the work group. Models evidence‑based practice and research with peers to improve patient care. Models effective resource utilization through the delegation of cost‑effective care delivery.
