Overview The Clinical Pharmacist - Home Infusion is an advanced practitioner in a specialized area of pharmacy practice responsible for dispensing and monitoring a variety of home infusion therapies, coordinating care with providers, and ensuring legal and regulatory compliance. The pharmacist is responsible for cost-effective delivery, consistent customer service, clinical effectiveness, patient safety, and overseeing sterile compounding and technicians as appropriate. Works collaboratively with an interdisciplinary care team to provide high quality, low-cost care in the patient\'s preferred setting. Minimum Qualifications California Pharmacist License eligible and possess a valid license at the time of hire PharmD, or equivalent training Three (3) years of recent pharmacist experience with infusion pharmacy or sterile compounding in a health system or specialty setting Ability to supervise technicians and establish priorities of workflow in an infusion pharmacy Interpersonal skills to establish and maintain positive professional relationships with other staff and patients Working knowledge of sterile products, IV additive preparation, and USP chapters 795, 797, and 800 Computer skills to facilitate electronic prescribing and physician order verification Working knowledge of basic clinical software programs Excellent written and oral communication skills Demonstrated problem solving, decision-making skills, and communication skills Preferred Qualifications Board Certified Sterile Compounding Pharmacist (BCSCP) Certification Board Certified Nutrition Support Pharmacist (BCNSP) Certification Home infusion experience Experience in health-system home infusion Experience developing, implementing, and improving policies and procedures Oncology experience Parenteral nutrition experience Working knowledge of Epic, Brightree, and/or AllScripts Working knowledge of acute discharge therapies (parenteral nutrition, antibiotics, cardiac medications etc.) Key Responsibilities 75% - Operational Duties 20% - Practicing Professional Duties and Development 5% - Other Department Overview UC Davis Health is a nationally recognized leader in the provision of high-quality, patient-centered care.
