Clinical Pharmacy Specialist (Oncology)

Summary Oncology Clinical Pharmacy Specialist (CPS) operates at the highest level of clinical practice, independently within an authorized scope of practice. The CPS collaborates with the Oncology Pharmacy Supervisor to maintain the program, provides direct patient care, and educates pharmacy staff. Qualifications Citizenship : United States citizenship; non‑citizens eligible only if no qualified citizen can be recruited. Education : Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited pharmacy college with a Bachelor of Science in Pharmacy (BS Pharm.) or a Doctor of Pharmacy (Pharm.D.) degree. Licensure : Full, current and unrestricted state license to practice pharmacy in the United States, including Puerto Rico and the District of Columbia. Licensure must be maintained if required by the state. License History : Applicants with any revoked, suspended, denied, restricted, limited, or probationary license must meet VA Handbook 5005 requirements. Licenses revoked for misconduct or incompetence are ineligible unless fully restored. Language Proficiency : Proficient in spoken and written English. Experience : GS‑13 level; requires at least one year of experience equivalent to the next lower grade (GS‑12). Preferably completion of PGY‑1 and PGY‑2 Oncology residency or at least two years of oncology experience. Professional Knowledge : Expert understanding of regulatory/quality standards, oncology pharmacy practice, and advanced drug therapy monitoring. Physical Requirements Heavy lifting up to 45 lbs; moderate lifting 15–44 lbs; light lifting under 15 lbs. Requires standing, walking, repeated bending, reaching, use of fingers, and near‑vision correction to 13–16 ″. Ability to distinguish basic colors, hearing sufficient for patient communication, and mental/emotional stability. Responsibilities Provide independent, mid‑level patient care, initiating, modifying, or discontinuing medication therapy and consulting on intensive medication therapy management. Design, implement, assess, monitor, and document therapeutic plans using evidence‑based, least‑toxic, and cost‑effective treatments. Perform physical assessments, order and interpret laboratory and diagnostic tests to evaluate medication efficacy and toxicity. Review inpatient and outpatient medication orders, including oncology drugs, for appropriateness and resolve drug‑related problems. Document recommendations and interactions in the electronic health record; complete medication reconciliation and provide updated medication lists to patients. Participate in adverse drug event reporting and document medication errors to improve patient safety. Act on Prior Authorization Drug Requests (PADRs), documenting approval or disapproval. Counsel patients and families on medication use, disease‑state management, and lifestyle modifications. Deliver drug information and resolve medication therapy questions for healthcare providers; provide education and training on oncology care. Maintain current literature and clinical knowledge to support continual professional development; train and mentor pharmacy residents and students as assigned. Assist in designing, conducting, and presenting medication use evaluations and research projects; collaborate on investigational drug studies and the compassionate use process. Maintain an independent practice scope under medical‑center leadership; serve as a mid‑level provider with autonomy over medication therapy decisions.
