Case Manager

Responsibilities Develop, deliver, implement, revise, and evaluate individualized patient‑centered care plans under physician supervision. Provide case management to an assigned group of patients and ancillary staff. Understand and submit required data collection sets (OASIS, PAR, etc.) and other documentation within required timeframes. Make initial evaluations and regularly reevaluate patients’ nursing needs per state and payor requirements. Advocate for patient welfare and coordinate care between patients, families, the agency, and other healthcare providers. Administer specialized nursing services according to competency level. Demonstrate problem‑solving abilities and positive interpersonal communication skills. Identify patient care preferences and verify that interventions meet goals of treatment. Initiate appropriate preventive and rehabilitative nursing procedures. Prepare clinical and progress notes for each patient visit and summary reports. Attend case conferences as assigned. Inform physicians and other personnel of changes in patient condition and needs. Counsel patients and families on meeting nursing and related needs. Educate on proper home health strategies and procedures, utilizing community resources as applicable. Observe and report patient symptoms, reactions to treatments, drug changes, and changes in physical or emotional condition. Reconcile medications, administer medications per agency policy and physician orders. Provide written instructions for aides or other ancillary disciplines caring for the patient. Process orders and notify physicians of patient needs and changes in condition. Refer cases to Physical Therapist, Speech‑Language Pathologist, Occupational Therapist, and Medical Social Worker when specialized skills are required. Adhere to policies, procedures, best practices, infection control, EMS, and compliance plans. Participate in clinical record/utilization review and quality assurance and performance improvement (QAPI) activities. Report all applicable infections, occurrences, and complaints. Submit documentation within 24 hours or as required by state regulations. Maintain confidentiality of patient, employee, and agency matters. Serve on‑call duty nights, weekends, and holidays; act as after‑hours supervising nurse and make nursing assessment and triage decisions under guidance of the clinical supervisor. Immediately report any patient incidents, variances, or complaints to the Nursing Supervisor. Demonstrate competent performance of technical skills according to established procedures. Maintain acceptable attendance status per agency policy. Report all incomplete work assignments to the Nursing Supervisor. Demonstrate effective time management skills through daily documentation and infrequent overtime for routine assignments. Ensure written communication of visit schedule, medication profile, treatments to be administered, other pertinent instructions, and agency contact information to patients/caregivers. Perform all other duties as assigned. Qualifications Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing. Minimum (1) year of nursing experience required; prior home health experience preferred. Average computer skills with competency in media communications. Physical abilities to perform the described duties, including transportation and travel as required. Organize work procedures through time management, assume responsibility, and effectively supervise ancillary disciplines as per state requirements. Apply professional training to critically think through patient needs, follow physician orders, and produce accurate records of nursing activity within agency guidelines. Accept the philosophy and goals of the agency. Exercise initiative, independent judgment, and maintain licensure and continuing education as applicable. Excellent verbal and written communication skills. Maintain health requirements for clearances, current CPR, and other certifications. Physical requirements: ability to write, dictate, or use a keyboard; utilize proper body mechanics in multiple environments; function in multiple environments. FLSA Status: Exempt.
