Choice Health at Home is seeking an experienced healthcare sales professional to join our growing team. Candidates must have prior healthcare sales experience with a proven ability to build referral relationships, drive growth, and expand market presence.
This is not an entry-level sales opportunity. We are looking for a motivated, relationship-focused professional with experience developing referral partnerships and generating admissions growth within healthcare environments.

What You’ll DoAs a Home Health Business Development Liaison, you will serve as a key ambassador for Choice Health at Home and drive strategic growth across the market.
Responsibilities include:
- Build and maintain referral relationships with physicians, hospitals, case managers, skilled nursing facilities, assisted living communities, and healthcare partners
- Promote Choice Health at Home services and position the agency as the preferred provider
- Identify, cultivate, and grow new referral opportunities
- Collaborate with clinical and operational leaders to support agency growth and patient access
- Support community outreach and market development initiatives
- Represent the agency at healthcare events and networking opportunities
- Maintain accurate CRM documentation and referral activity tracking
- Meet and exceed monthly admission and growth goals
- Minimum of 2 years of healthcare sales experience REQUIRED
- Preferred backgrounds include Home Health, Hospice, Physician Relations, Healthcare Business Development, or Healthcare Account Management
- Proven success in building referral relationships and achieving growth targets
- Excellent communication, presentation, and relationship-building skills
- Ability to work independently and manage a territory
- CRM experience preferred
- HomeCare HomeBase experience preferred
- Valid Driver’s License
